I work part-time at a small clinic's front desk, where I sometimes help file paperwork. Last week, I was alphabetizing folders when I noticed my close friend's name on a file near the top of the pile. At first, I just felt surprised seeing it, since they've never mentioned being a patient here. As I absentmindedly stacked the folders, the top sheet of their file slid out, flashing a diagnosis—something serious and chronic, but potentially manageable if treated: Type 1 diabetes. My heart sank. My friend and I talk all the time, and never once have they mentioned any health concerns. In fact, they rarely seem to do anything to take care of themselves and sometimes even joke about living off soda and junk food. That explains a lot, suddenly. It's possible they are still in denial, or maybe they just want to keep this private. But what if they’re not managing it? What if their health is at risk, and I could help by speaking up? Legally, I’m not supposed to know any of this—HIPAA and all that. But emotionally, it’s eating me up inside. Should I say something and potentially betray their privacy? Should I stay silent and trust them to handle it if and how they want? Or maybe I could gently nudge a conversation without saying I know anything, but even that feels a bit dishonest. I feel anxious every time we hang out now, scared for their health but also guilty for knowing something I shouldn’t. What would YOU do?
