I found out last week that my younger brother, who’s only 32, has been secretly skipping his dialysis sessions. I caught on because he slipped up about times and seemed sicker than usual. When I confronted him privately, he broke down and told me how exhausted and hopeless he feels. He doesn’t want anyone else to know—not our parents, not his fiancée, and especially not his doctor. He made me promise I wouldn’t say anything. He says it’s his body and his choice, and that he’s tired of living in and out of hospitals. But I know from the little research I’ve done that skipping treatment could kill him. Part of me wants to honor his wishes, respect his autonomy, and support him no matter what. Another part of me feels like I’m silently helping him die if I keep his secret. Our parents would be completely devastated if anything happened to him, and his fiancée has already lost someone close to kidney failure once—she would never forgive me for saying nothing. I lie awake at night, feeling like every day I don’t tell someone, I’m gambling with his life. But if I break my promise, I could destroy the trust we have, and maybe even ruin his only source of comfort. I don’t know if I should protect his choice, betray it to save him, or find some possible middle ground. What would YOU do?
